2000-01 was almost a lost season for Hartlepool United's highly rated young central defender.
In the early weeks of the campaign, he was kept out of the first team by a knee injury, then after the briefest of returns in September, further complications set in and he required surgery.
Determined not to give up, he recovered sufficiently to return to the first team in April and he played in the last seven games of the season.
Graeme's contract at Sheffield Wednesday was extended to June 2007 in June 2005.
Graeme joined Doncaster Rovers in January 2006 with a contract to June 2008.
He joined Bradford City in July 2008 with a one-year contract.
Graeme joined Notts County in July 2009 in a two-year deal.
